NEW Patient Representation Group

Commitment
Two meetings per year to discuss our provision and manner of delivery of services. Meetings will take place on a weekday, in the evening.

Agenda
August 2011: Discuss which issues are a priority for practice development and improvement, to be incorporated into a practice survey (e.g. the extension of opening hours, and facilitating access to other health and social care providers).

January 2012: Discuss and comment on the findings of the practice survey, agree on changes relating to the provision and manner of delivery of services, and develop an action plan of priorities and proposals.

Impartiality
Meetings will be facilitated by an individual who is not a member of the practice.

Accessibility
We endeavour to create an inclusive, representative group. Meetings will be held in the surgery, which is accessible for wheelchair users. We will enable representation in conference/Skype form for the housebound if feasible. If you have any other requirements, please let us know.

Suggestions? Please let us know.

To join, please leave your name, contact number and availability at the reception (tel: 020 7730 5171).
